What to Expect
Our exams follow the strict 49 CFR 391.41 federal standards required for all Idaho CDL holders.
The Idaho DOT Exam
- Vision & Hearing: Minimum 20/40 acuity and the ability to hear a “forced whisper” at 5 feet.
- Blood Pressure Screening: Federal guidelines allow for certifications ranging from 3 months to 2 years depending on your readings.
- Urinalysis: A kidney screen (not a drug test) to check for underlying conditions like diabetes.
- Physical Evaluation: A 12-point assessment of your respiratory, neurological, and musculoskeletal health.
2026 Electronic Reporting Update
As of 2026, Idaho has transitioned to the National Registry II (NRII) system.
- Electronic Submission: Our office electronically transmits your results to the National Registry within 72 hours.
- Paper Backup: Per current FMCSA waivers (valid through April 2026), we still provide you with a physical Form MCSA-5876 (Medical Examiner’s Certificate). You must carry this paper or a digital copy on your person for 60 days as proof of certification while the state systems sync.
Idaho CDL Requirements & Self-Certification
In Idaho, all commercial drivers must self-certify the type of commercial operation they are engaged in. We help you ensure your medical card matches your specific driving category:
- Interstate Non-Excepted: You drive across state lines and must meet Federal DOT medical requirements.
- Intrastate Non-Excepted: You drive only within Idaho but are still required to maintain a DOT medical card (e.g., hauling non-exempt commodities).
- Active Status: If your medical card expires, ITD will automatically downgrade your CDL to a Class D license. We provide the rapid exams needed to prevent this.
Required Documentation for Idaho Drivers
To ensure you receive your medical card on the day of your visit, please bring:
- Current Idaho CDL or Driver’s License.
- Medication List: Including dosages and prescribing doctor information.
- Diabetes Records: If you are insulin-treated, you must bring the MCSA-5870 form completed by your treating clinician within the last 45 days.
- CPAP Compliance: If you have sleep apnea, bring a 90-day usage report from your machine.
- Specialist Letters: For heart conditions or recent surgeries, bring a clearance letter from your specialist.
